To register, we ask that all patients use our online registration applications. If you need paper forms, feel free to visit the Surgery and collect the forms from our Reception. If you have any problems completing the digital forms, please call the Surgery on 01386 443333.
There are some restrictions on registration for visitors from abroad. For more information please visit NHS For Visitors From Abroad.
The practice has an open list and welcomes new patients.
To register with the practice you will need the following:
Proof of identification which will include seeing photographic evidence and proof of your address.
If you are unsure about any of the forms you need to complete or do not understand them, our reception staff will be happy to help you.
On registering with the Practice, new patients will be asked to complete a questionnaire. The questionnaire provides us with some background information about you including medical history and any allergies.
If you have a condition that requires regular attention, please mention this to the receptionist when registering, so we can then request your medical notes more rapidly from your previous surgery.
